During the initial phone call I will ask you basic demographic information and may request that a payment card be put on file.

Insurance is accepted for individual therapy services. During the initial phone call your insurance information may be gathered to check your eligibility and benefits.  I accept most out of network policies and offer a Superbill for you to submit for reimbursement if your policy covers provided services.

Before our initial meeting, you will be provided with a welcome packet, including information on my practice policies and fees.
